Confirm the patient's identity with the patient, all notes, and wrist band. Operating Surgeon, or the nominated deputy who will be present in theatre, during the procedure.

Mark the surgical site with an arrow, using an indelible pen. Operating Surgeon, or the nominated deputy who will be present in theatre, during the procedure.
Before leaving the ward or day care centre, the mark is inspected and confirmed against supporting documentation. Ward or day care staff.
Any imaging data should accompany the patient, or such imaging should be available in the theatre suite. Ward or day care staff.
In the Anaesthetic room, and prior to Anaesthesia, the mark is verified against the patient's supporting documentation. Operating Surgeon, or a senior member of the team.
Ensure imaging details are available in theatre. Operating Surgeon, or a senior member of the team.
Ensure any relevant implants are available. Operating Surgeon, or a senior member of the team.
The complete team pause to verbally confirm the correct patient, site, and procedure. Theatre staff directly involved in the procedure.
